Rough rough idle

Rough rough idle

On my pickup, which has over 200k has developed a very rough idle and isses sometimes, its had a complete tune-up recently, so its nothing of that matter, im wondering if the timing chain has stretched and caused it to give it a rough idle, i heard it was suppose to be changed every 150k, whats involved int he process.
