Did it ever do that , These shifts are controled by the govenor at full throttle , You may be able to get there manually which contrary to popular belief will not hurt anything,

Like was mentioned before, it depends on the tranny governer (controls upshift and downshift points based on transmission speed), and therefore gearing of the car. A pickup with a 2.73 rear end isn't gonna be turning as much rpm at 20mph as a F-body with a 3.42 or whatever rear end. To add to this, the 700R4 has a termendously high ratio (low speed, high rpm) 3.06 to 1 first gear. I know my car will not hit first at much over 15 mph, I'm running a 3.73 rear end at first gear at 20mph is like 3700rpm. One the oposite end of things, I've got a 99 Grand Am with a 3.08 final drive and a much lower ratio first gear that'll shift down to first at anything under 38mph, and pull first gear to 50mph. (It does have a modded PCM) So each car will be different. If the TV cable is properly adjusted, you'll get the maximum downshift speed thats reasonable.
