305 TBI rough idle

Here's a video of my car on a cold start. Lots of surging and almost stalling out. After a few minutes it smooths out for the most part, but it takes a while. Any ideas what is causing this?

If...

Exhaust leak at header gasket?

I'm pretty sure I have an exhaust leak. I can hear it when the engine is running, and it gets more intense as I give it more gas. I can also smell gas/fumes a pretty decent amount.

I looked last...

Re: Hot rear wheel and burning smell

It turns, and of course the drums/shoes are (hopefully) just barely touching, so it takes a little effort. I think it turns OK, but not sure how easy it should be.
