code 45 when idling

Well spring finally here.. and I start my truck and ran it for about 10 mins. I get the code 45. this code has been with me for about 3 years now.. 2 summers ago I had the engine all rebuilt with mods in the sig. the code still comes up.. it only ever comes up if it's been idling for a while. yet before I put it away for the winter and removed the plugs (I believe champion rc12yc). thye looked a little on the lean side. could it be the injectors causing this? I think they are original from 88. I have a set waiting for me to pickup accross the border. they have been reverse flowed/cleaned and matched up with 1-3 %.. I'm hoping this may solve the problem.
