Fender Removal, 1 bolt left?

I've removed all the bolts from the fender except one. Its at the bottom of the fender near the passenger door. I can't use a socket wrench to remove it because all I can see is a round piece of plastic (looks like a washer) with three circular holes in a triangular pattern. I tried to locate the other end of the bolt by checking in the inside of the car underneath the carpet but still didn't find the other end of it. I don't know how to remove it and I've already tried forcing it out but no luck so far. Just this one damn bolt is frustrating me. Any help will be appreciated.

Hi.
Sounds to me as if someone has sheared the head off the bolt that should be there.
The bolt goes in horizontally from the outside and threads through that washer looking thing through the rocker panel flange. You should be able to see the threads from underneath.
Drilling through it may be the only option.

The bolt is fine its just that I think the previous owner lost the nut and jammed it some plastic thing to make it similar to a "push pin". I've even tried to melt it off, nothing.

This bolt I've been having trouble with shouldn't have been where it's currently located. I'm assuming that the previous jacked up the car improperly and bent another bolt, causing it to pinch between the fender.

The first image I attached shows how the bolt looked as I saw it originally, the second image is after trying to bend the bolt back to its place. Third image shows how it's pinching the fender.

Mine was the same way and was bowing out the fender. Grabbed some vise grips, locked onto the sheet metal and bent that sucker straight down.

I couldn't find a vice grip so I used a hammer instead. I swear this was pissing me off, I couldn't figure wtf was wrong till I checked with the driver side bolts after 2 days. lol

I removed my DS fender today and this POS bolt caused me to skin part of my left pointer finger and my right thumb... ouch!! At first when I looked at it, it looked like a round bolt that had to be removed with a special tool that goes in the 3 holes which will allow you to turn it. I ended up finding out that the fender tab which this bolt goes in through was bent outwards enough (most likely from a car jack) hiding the actual hexagonal bolt head. There I was like an idiot trying to remove that welded 'round bolt' with vice grips which is actually what the bolt (hexagonal) screws into. Just my if anyone encounters and is confused about this 'bolt'.
