what makes upper door hinges special

These things are pretty expensive for what they are WHY? Is what makes them special just that they clamp the hinges together like a nut and bolt.

For those of you with greaseable hinges how does the grease work. Is it just a hole down the middle of the bolt and then a hole that passes through and let's the grease exit inside the bushing.
