3 quick questions about over-heating

1. If BOTH fans ARE running and the car still overheats... what could the problem be? Is it even possible?

2. When setting at an idle, the temperature gauge goes up (way past normal) and then comes down slightly... over and over. What could be causing this? The fan(s) kicking on and off?

3. My coolant level is always at the Cold level in the morning and the Hot level whenever I drive it... however, today my guage went to like 250 and when I checked it... the coolant was 4 inches above the Hot level and the top of the overflow container and the cap had coolant on it? What does that mean?

I replaced my CTS and it didn't help. What else controls when the fans kick on? The thermostat?

Could it be a wiring or relay problem?
