6E Torque Converter Logic

The tables for torque converter logic in $6E are a bit confusing to me. I have a few questions listed below.

1. With stock TCC logic, is the converter EVER locked when at WOT?
2. If it doesn't lock under WOT, would it be beneficial for both temps and power transfer to make it lock (and how would I go about doing that)?
3. On the highway I don't always get it to lock while I'm coasting or just doing general highway driving unless I use the cruise control. I would like it to lock on the highway to keep temps lower and improve my fuel efficiency. How do I get it to consistently lock?
