This is what happens when the caliper carrier to adapter bracket bolts back out on you at 30 MPH or so. I've only heard of this happening to a car once ever (bolts backing out at speed) and can easily be prevented by using lockwire or safety wire on the heads of the bolts. I used lockwire on my 1LE car just for that reason.
Here's a couple links to how to install lockwire. All you need is to drill small holes in the heads of the bolts you install and wire them together as the links describe. In the case of the LS1 brackets like Jeff has, drill a small hole near each carrier bolt and wire the bolt to that hole.

Summit Racing supplies safety wire, and there's a ton of safety wire pliers on Ebay in the tools section and through racing suppliers like Lockhart-Philips/USA.

We all install upgrades and sometimes don't take all the precautions we can to ensure they're installed as well as they can be. Hell, I didn't lockwire the bolts on my most recent C4 HD upgrade but you can bet I will be just as soon as I find the time to do it. I trust myself, and I'm sure Jeff did, but sometimes things just happen.

Factory bolts, yes, but these aren't factory bolts. The factory LS1 bolts are too long for the 5/16" and 3/8" brackets.
