From my research, most of the companies use the Afco sleeves.
However, A word of caution, they are paper thin. They are made of .065 wall tubing before the threads are cut into them. I do know of one breaking first hand from a person that had it happen.
This info was given directly to Steve Spohn hence why Spohn has opted to ciuston CNC his own sleeves and threaded adjuster nuts to a perfect fit on our third gen struts rather that using a universdal one that is thin and has slop fit onto a 3rd gen strut.
IF you have a problem with a sleeve, it is good customer service for the vendor you bought your kit from to replace yours without the cost of buying an entire new kit- they have records of you buying one in the first place. Now if you only want the sleeve because you want to build your own from scratch, then you are most likely SOL and will have to measure and have a machine shop make two custom sleeves and nuts for you at about $400-500.

Thanks for the info,I did want to build my own from scratch. Also I looked at the AFCO site and they dont show a size that will fit over our struts I measured 2.18 OD on KYB GR2's.As you say Im SOL
i had mine cnc'd, much better in the end. you'll want 2.5" id for most struts and 3" for bilstein or larger setups. 2.5" is a very standard size for spring choices when it's time for you to shop, something to think about.

Hm I'm pretty sure you can buy the threaded sleeves separately because I was at a guy's house a few days ago that had gotten an extra pair (one of his first pair got stripped). I'd considered building my own setup too, but I'm not sure how "worth it" it really is when you can jsut go and buy a setup pre-made for $400.
