My friend has to run thick oil in his engine what does this mean??

"Thick" oil generally means higher viscosity oil. If your friend has determined that "thick" oil is necessary, it may be due to worn bearings which allow internal leakage and prevent the oil pressure from building to a safe level. It could also mean that piston rings are worn to the point of allowing excessive oil into the combustion chamber, and the more viscous oil would tend not to blow by as easily.

It could also mean that there is excessive external leakage and the more viscous oil would tend not to run out as easily.
