Throwout Bearing wear

I replaced the clutch, press. plate and throwout bearing In my Iroc less then 800 miles ago. Also had the flywheel resurfaced. I am repowering now and pulled everything out and removed the tranny again too. The throwout bearing seems very worn. It was also making a squeaking noise when car was idleing in neutral. I didn't dog the clutch or power shift any... Could I have installed something wrong? I know the large end of the bearing was facing the correct direction. I am not sure I installed the new pilot bushing all the way in to seat properly tho. Could this affect anything? Thanks!

if you're sure you had it installed correctly i'd replace it. if you never had it installed correctly i'd replace it too. just recheck that you did have it right.
