Air Fuel ratio to rich - no errors

Some time ago I started to smell a lot of fuel from the exhaust. Fuel consumption went up, and You can hear fuel burning in exhaust after taking leg off the the throttle pedal.

So I've checked error codes -nothing
I've disconnected O2 sensor, drove for some time - still no errors

So I've checked wire form O2 to ECM, and everything looks all right...
Checked everything on three different O2 sensors.

Next step was to replace ECM, and still no codes, what is going on?
And it was even worse bucause cars started coughing on second ECM

I have EGR and Air pump deleted frome the car and from first ECM, and on the second ECM there is no error codes about EGR... still don't understand it.

It's only showing code 12 on both ECM's

This is crazy!

Couple months ago I've raplaced injectors and fuel pressure regulator-non adjustable but in this department everything is OK

Have you checked fuel pressure watch for a drastic pressure drop after prime ?
