how can an adjustable fule pressure regulator break?

What would cause an adjustable fuel pressure regulator not to work and would it not work all the time or just sometimes?

The diaphram inside could crack or rip

Yeah, if the diaphragm goes, that regulator is toast. You'd be running at the WOT throttle setting all the time.
