Just that it's in sound condition with no missing parts. Especially if it has different brakes (would suck to be missing a caliper bolt and you can't pull one off of your original rear because you have drums..)..
If it's practical, I'd say to definitely remove the diff. cover and an axle to inspect the posi unit, gear teeth, and axle splines. This way, you can also visually verify the gear ratio (by counting teeth) and axle apline count in addition to just their physical condition.
If it's still on the car, take a peek at the odometer while you're at it. Even though they can last a good long while, if you end up with several choices, it's nice to be able to pick one with fewer miles on it.
Check for bent, broken, or stripped wheel studs and control arm mounts.
Now this is being extremely thorough. Most of the time you really only have to worry about a posi unit being toasted from the getgo.
