Holley 4160 Metering Block

Doing a rebuild. The vac port nipple came out of the block when I pulled/twisted on the hose. Also, one mixture screw seems as if it goes in further than the other (perhaps a heavy handed goof in the past). Given that into should the block be replaced?

Those ports often come out; they're just a light press fit. You can just poke it back up in there, maybe tap it GENTLY with a hammer to set it in. Maybe run a sheet metal screw up into the tube SLIGHTLY first, to SLIGHTLY expand it, to tighten it up. (emphasis on SLIGHTLY)

The idle screw seats are often not symmetrical. You can look at the screws themselves, and look down in there at the seats and see if they're damaged, and go from there.

Many thanks. While I have your ear, is the secondary shaft supposed to be tight in its housing? Mine wiggles a bit.

They're never really "tight"; but they shouldn't just bang around. Newer ones have the green Teflon coating on the shaft, so they don't wear out as quick as they used to. The sec doesn't wear out near as much as the pri anyway. (well, not for most people, at least...)
