PROM Identification

I bought a used MEMCAL to modify so that I can burn my own EPROM. Can some one please Identify it for me? The guy I bought it off of said it was for a 1986 LB9.

Side of chip has #'s 16035006 8604

Chip under blue cover has 8980ANY on the sticker.

Any help is appreciated.
