Scanning software shows REALLY rich condition!

I just connected my laptop tonight and during cold start up the O2 sensor was reading about 900 mv, and the displayed a/f ratio was about 10.5 and slowly droped to 11. Makes sence since I have the SVO injectors set to 25 lbs, at about 45 psi. But once it warmed up it still showed a rich condition, the O2 dropped to about 700 to 800 mv and the displayed a/f ratio not only stopped, but it stayed at 11.99 and didn't fluctuate. What gives, is the O2 senor bad... it is only 1 week old. Is the formula in Craig's software corrupted somehow? I know it is rich, but really 11.99:1 I always thaught 700 to 800 mv was closer to 13 or so.
