Question about timing advance?

Which is better, mechanically set the 4-6* advance timing with a light and the EST disconnected and make further changes in the prom or set the timing to TDC mechanically and then make adjustments to the base timing in the prom?

I could never get my car started with base timing at 0*. ESC takes over at 400 rpm. 0-400 is off base timing.
