subwoofer stopped working...

Out of the blue my subwoofer stopped working. I've got an inline fuse between the battery and the amp, and another fuse on the amp. Both are good. I checked voltage at the amp itself and it was showing over 16v. Now sometimes the wires would come loose on the sub box and stop working, so I'd just reattach and tighten them. I'm wondering if something inside the box came apart. I installed this with a friends help like four years ago, and I don't even remember how the wiring works Should I have over 16v to the subwoofer? What has been the problem if this happened to you, other than what I mentioned?

I don't know how to tell if the amp is on, it has no lights or anything. It has power going to it for sure, but I don't have time to really troubleshoot it until winter break(midterms next week and the week after, then final projects and finals). Thanks for the response.
