I sure wish this had been a sticky, but now that I have figured it out, here are the difinitive p/n's that I needed to make my headlights operable again.
Isolation relay - GM p/n 100226220
Headlamp motor relays (two of these, one for each headlight) - GM p/n 22535475
I hope this helps those of you that had as much trouble as I did while trying to make my flips operate again.
