Driverside window keeps tearing up the outside window trim

My driverside window keeps tearing up my outside trim on the door and pulling the inside trim piece off the door panel. The window isnt off track but acts like its coming up at the

wrong angle, its also very loose like it missing a guide or something. Everything seems to be in place (felt and track, but it acts like its out of place. Any insight on this?

Could be the outer trim piece is missing the screws to hold it inplace & the inner trim is broken off the door panel itself. That said, how does the glass seal against the weather-strip?

The outer piece keeps getting ripped off the door. Ive replace all the screws for the outer piece but when i roll the window, it comes up under the outer trim piece instead of under and beside it. Like it literally comes up directly beneath it as if the window is somehow out of track but ive looked, its still on its track, just wobbly. It seals fine as long as i push the trim back against it after rolling the window up.
