Best bet would be to get a nice square photo or scan, then crop, edit the colors to be true, and then print a copy on cardstock. Laminate your repro and you're good to go.
I was just thinking that might be the only realistic route as I've been unable to get anywhere locating one. I'll see if I can get a scan of one sent to me. Thanks Drew.
What code are you looking for? Its also stamped on the side of the prop valve if you want the original code. Also, I have seen 2 different ways that the center was cut out.

When I gutted and cleaned my six'r, I found the original tag caught under the brake lines on the crossmember. Worth looking around, it might still be there somewhere.

Thought about that last night so I crawled around as best I could but found nothing. I even walked the concrete slab where I worked on it as well as the surrounding vegetation, nothing. She's traveled about 1000 miles on the back of an open car hauler since I lost the tag so it's probably laying on the side of some highway somewhere in Texas.
Well I finally got around to addressing this lost prop valve tag. I printed the actual pic of the tag adjusting the size based on the size of the prop valve installed in my car. I then laminated it and cut out the round center section. I'll run with this for now until I find an actual tag.
