Codes and hard start

Transmission: T56
Axle/Gears: 3.70 9 bolt
Codes and hard start
Yesterday I finally checked what codes were being tripped in my car. Sometime after it warms up (I am on the highway by this time), the SES light goes on and off randomly, mostly staying on. when I checked the codes I got a 12, 13 and 34. I know that all the 12 basically means that the computer is hooked up, what do the other two mean? On saturday I checked for the codes, but since the engine was dead cold all I got was the 12.
This morning when I went out to go to work it cranked over fine but wouldn't fire. It would only fire once i gave it a little gas, then it wouldn't idle without me pushing gas pedal a little. After about 1.5 - 2 miles it would idle, but at only 450 - 500 rpm(thermostat @~ 140/150). Once I got to work (30 miles later) it was idleing back at normal ~ 700rpm in P or N. This is the first time this engine hasn't fired up right away and run strong from then on.
Basically I have 3 questions:
a) what do the codes mean,
b) what could be causing my car to not run right,
c) are these problems related?

Code 13 Oxygen sensor circuit failed
Code 34 MAF sensor voltage reading too low or no signal
Check the female pins on the MAF connector, make sure they aren't spread apart.
Change the O2 sensor if you haven't reciently.

Wow, good thing I bought that senor prob, it'll be seeing duty a lot sooner than I thought it would. Thanks for the help.
Anybody think that these two problems are related? sounds plausible to me.
