Carbon Build-up on Oxygen Sensor???

What's causing my Oxygen Sensor to have Carbon build-up? This is the second time I cleaned it....

incomplete combustion maybe, too rich or burning oil, does it set a code or you jkust checking and cleaning it?

Replace the sensor, its an easy $25 job.

As far as the buildup, check for leaking injectors, oil, coolant. Anything that leaks on the sensor could ruin it.
