Alt belt turned itself inside out

Hi everyone!
I'm new here. Well I'm new to posting.
Anyway, I have a 1984 Recaro T/A (L69).
The belts needed replacing when I bought it. Easy enough, right?
I opted not to spin the AC as it needs recharging (at the least) and windows roll down for a reason.
So I now had a 3 belt setup. Except that the alternator belt flew off two weeks ago, and took with it the smog pump belt.
After that I replaced the alternator/water pump belt (again) and removed the smog pump, so I'm down to a 2 belt setup. Power Steering, and one for the alternator/water pump.
Today I noticed that the volt meter was dipping low when I ran the fans (AC - just for air movement inside). I popped the hood, and to my surprise, the alternator/water pump belt has it's teeth pointing out! It flipped itself inside out!
How is this possible? I checked the tension yesterday! I must be doing something wrong, but what?
Any help would be great. Hell, even a "moron, you did _____ wrong!" would help!

Aren't those vbelts set up to where they line up to eachother? It would sound to me like a belt would have no trouble flipping/coming off like that. Then again, I could be wrong.

Not sure about the alignment being the issue or not.
Could my pulleys be worn out? On the water pump or on the crank I mean.
