PS pulley wobble. Time for a new one?

My power steering pulley is wobbling pretty bad. Is it time to replace it? The hex nut is a tight as I could get it

The hex nut doesn't effect how tight the pully is mounted. It's either bent, pully coming off shaft, or the bearings have gone. Just replace it.

Mine's been like that for as long as I owned the car. 4 years? I guess I'm not too concerned.

Ok so it should be safe to drive for a few days while I wait for the new one? The last thing I want is that flying out of the hood

Well, It matters why it is wobbling of course. I think the previous owner, or somebody tried to unsuccessfully pry it off. Mine isn't going anywhere, but if your pulley just started flying south, I would get to the bottom of your issue asap.
