Low voltage, once drops doesn't come back up...

Low voltage, once drops doesn't come back up...
Alright, for all you search flamers out there:
I did a search and everyone was looking at the stock voltage gauge inside the car, and it didn't quite match my problem...
My car will run at about 13.5v13.8-v right at startup, it stays about the same the whole time. Once warmed up, it drops to high 12v. Once it hits 195* (where my chip turns both fans on), the voltage dips, and used to return. Now it just stays at very low 12v when at a light, but returns to 13v under throttle. Even when the fans turn off at 185*, it stays that low.
Tonight I was just cruising, atmospheric temp was mid-80s, and high humidity. The car was running at 178* and still was that low of voltage. Alternator is new, got it last November. Battery reads 12.5-12.9 depending on the day and heat.
The mods are in the list, and I have a gauge that uses a relay from the key. The relay connects the voltage gauge directly to the battery.
What should I look at now?
