injector failed mechanically, stuck open
I recently swapped out the original injectors on my low mile showroom condition all original 1990 vette because there were some idle quality issues with the old ones. I got these bosch IIIs because I have them in some other cars also and they perform excellent in these so the choice was easily made.
Anyway... a little tinkering and they were installed. Engine ran fine and had a silky smooth idle.
1st test drive however on the way home I noticed a little fuel smell from the exhaust. Only a couple miles to home so no biggie.. it started smelling a lot worse and eventualloy started to smoke like crazy so I shut it down. I was doing disgnostics with a scanner to see what would cause it to run smelly rich but couldn't find anything wrong and there was no real indication of a controlled rich condition. The white smoke made me very nervous so I shut it down, only to have the engine seize violently after turning over a little. WTF? sounds like it locked.. so I started to pry a little deeper and noticed that with the inj. fuses removed I could hear a toilet flushing sound through the throttle body (opened fully) ... how can that be. So I pulled all the injector leads, checked again... still the flushign sound. WTF?? So I pulled the fuel pump fuse and it was gone. I started pulling spark plugs and sure enough #4 was wet and gas was running out the spark plug hole. I hope there's no bent rod or even worse a cracked cylinder from the obvious lockup.
So, I have to at least replace that injector...but has anyone here ever had this happen???

That sure is a lot of fuel and I hope the white smoke is excess fuel turned into a lot of water vapor by the cat cons... If the engine has a cracked cylinder I will really be in tears

I have. A set of Mototron 60's (Siemens Deka), were cleaned the previous year and re-installed into the engine. It came on suddenly where the engine wasn't running right. A lot of popping and banging in the exhaust with the engine running rough.
Tracked it down to the injector on #4, replaced all of them with new injectors (brand new, not recycled ones).
Cut the offending injector open and found a piece of debris in the pintle/seat area. It was like a piece of slightly gooey plastic, have no idea how it could have gotten into that area of the injector.
I figured it was either: something in the fuel precipitated out and formed the piece of debris. Or, when the injectors were cleaned it was forced into the injector at some point when the filter basket was not in place.

Lost an engine myself not too long ago due to that very same thing, brand new injector somehow had debris in it. At this point, I don't care if the injectors are new, or used with low miles, I always remove the basket and clean them with a purge/trigger kit that I put together, especially when I sell a set of injectors, I will do the same thing for the buyer...

Worst case: The cause of the lockup might be the No. 4 cylinder was washed of engine oil from all that fuel and seized.

Not exactly the same, but I also had a brand new injector leak. When shut down hot, I could hear a sound coming from #5 that would increase and then gradually diminish to nothing. Turned out the injector was leaking when hot, but not in the volume you exerienced.
