pilot bushing wasted, what happened?

pilot bushing wasted, what happened?
i pulled the manual trans on my 88 2.8 camaro and the pilot bushing had come apart and pieces of are "welded" to the input shaft of the trans. its completely gone on one side of it and worn way down on the other side. it wasnt even in the crankshaft, it just fell out of it. any ideas what happened to cause this?

Ran dry.
When you put the next one in, make sure you pack plenty of good, high-quality high-temp grease behind it. I prefer either moly, or synthetic.
You could also use a bearing instead.

wouldnt it be pretty easy for that to run dry though? its not sealed off or anything really. and when i asked about it, they said its only a bushing, no bearing. i originally thought it was a bearing but i guess there isnt a bearing available, just a metal bushing.
