clutch fork isn't aligned?

my clutch fork seems to be off about a half inch to the outside, as in the ball on the slave cylinder doesnt line up with the bump its supposed to go in and the fork doesnt line up with the clutch hole, so i cant center the throwout bearing. whats up with this and how can i fix it? does it have to do with the hex bolt that appears to be the pivot point of that assembly?
thanks, Adam
