rear drum to disc swap - is it posible

Hi, I need to swap a drum brake rear end to a disc brake setup. is it posible and is it difficult? please help

FYI, ebmiller88 is fabbing up a backing plate that will allow you to forgo modding the axel flange as demonstrated in the link. Search his name.
