All T56 swappers...need help!!

All T56 swappers...need help!!
OK, I have not started my swap yet, but I am trying to get a picture of what I am going to do before I start, so I don't screw up.
1)How exactly does the hydrolic assembly go together. Does the master cylinder bolt to the fire wall, or where does it and the slave cylinder go?
2)Is the clutch pedal attached to the master cylinder or what does it attach to?
3)How and where does the clutch for install? In the article I am reading about the swap, it doesn't mention the clutch fork. Help!!
