Why does my transmission do this?

Transmission: T56, 700R4
Axle/Gears: 3.23 Torsen, 2.73 Posi
Why does my transmission do this?

A few months ago I got in my car put it in drive and pulled out. As I went I noticed that it was revving really high and refusing to shift into second. When I stopped I turned the car off and cranked it back up. This time though I had foot on the accelerator so it revved high right when it cranked. I put it in drive and it shifted fine.
Now every time I crank my car I have to hold the accelerator down as it cranks and it will shift fine. If I don't do this though the car won't shift from first. What could be causing this?
I know my transmission's due for a rebuild because I'm slowly losing OD but I didn't see any mention of this problem on the forums.

I'm not an expert, but the only thing I can think of is an improperly adjusted kick-down cable, its the only thing that has to do with the throttle as far as I know
