not shifting into 4th
Is there any difference in when the other shifts occur since this started? Are they later too? The first step should be to check and adjust the TV Cable: To adjust the TV Cable skip the book method and do this: Press the D button on the adjuster and loosen the cable setting. Now, have someone floor the gas pedal while you check to see if the throttle plate is open all the way. Usually they are off a little and will wiggle. Do whatever you have to do to get it open solid. Now, with the pedal floored, press the D button and pull the cable back as hard as you can and release the button. You now have the engine, tranny and gas pedal all in sync at WOT. Mark the cable so if for some reason it gets moved you can know where to put it again. The problem with the 'self adjust method' is that it bends things and puts them permanently out of whack. It is also the full TV setting, which is theoretically what the GM method does.

What it is doing is, 1-2 change is happening at about the right time, although a little bit harsh when cold.
2-3 shift seems a bit delayed, but is happening very smoothly, and i can only just feel the TC locking up at about 40-45 mph in 3rd. (RPM only drops about 1-200 RPM)
3-4 shift isnt happening at all, even when on the freeway at about 65mph, its still in 3rd, doing about 2750 RPM.
Moving the shift lever from overdrive to drive makes no difference at all.
If the TV adjust doesnt work, does anyone else have any ideas please?

Checked the TV cable today, everything was ok.
However one thing i did find was that while cruising in Overdrive (trans is still in 3rd though) if you lift of the accelerator, the RPM drop to near idle, get back on the gas and 3rd is engaged again.
Cruising in Drive, 3rd is kept, whether on the gas or not.
So, off the gas in Overdrive, and it goes into neutral...back on, 3rd is engaged again.
However one thing i did find was that while cruising in Overdrive (trans is still in 3rd though) if you lift of the accelerator, the RPM drop to near idle, get back on the gas and 3rd is engaged again.

So, off the gas in Overdrive, and it goes into neutral...back on, 3rd is engaged again.
If someone did any work to the tranny it is possible that there is something wrong with the Teflon seal on the O/D piston. That is the piston that goes into the Servo cover.
Since you didn't indicate that then there is another possibility that you are getting some spring fade a losing calibration. I have seen this happen and cause no 4th. What happens is prerssure in the governor circuit gets low and is never in sync with the 3-4 shift valve, keeping it out of fourth. Of course there could be something hanging the 3-4 shift valve, 3-4 relay valve, or 3-4 sequence valve (although you usually have a no reverse problem if one of those last two is stuck (I forget which now).
The quickest fix in that case is to install a TransGo Shift Kit since it will replace key springs and restore/correct calibration.
