Clutch failure need opinion on what happened

Engine: Built 350 sixty over
Transmission: W/C T-5
Axle/Gears: 4:56s auburn posi, mosers, girdle
Clutch failure need opinion on what happened

I have a centerforce dual friction behind my 350 with a t5. The car ran 12.5 at 112 with little traction. Anyway the car seemed to pick up an engine vibration and would lurch forward while in first and the clutch disengaged. so i started investigating and found that my pressue plate was screwed. There are 3 bands that connect the surface plate to the outside shell of the pressure plate. The were all bent in a somewhat V shape, one more than the rest causing the two halves to be off center causing my vibration. The weird part is the way they are bent it would have to be on the decel side but the car is never engine braked or put under stress in that direction. Any ideas of how this could be caused.
