Depends how much crap is in it. It could ruin the trans eventually if it gets dirty enough. Usually it doesn't though. The trans fluid starts to break down with time like oil so it is a good idea to change it every once in awhile. The fluid is very important in an automatic.

Slipping and overall consistency and performance of the transmission can be effected. Are you having a particular problem with the trans? Sometimes it's a fluid problem and sometimes it's a hard part problem. What color is you fluid? Is it still bright red?

The fluid is a brownish-red, burnt orange.
The problem with my tranny: My car, when the drivetrain is under heavy load (going up hills and what not) my car will start to shake pretty hard.

The first thing I would do then is buy a filter kit for the trans and some new fluid. The filter kit comes with a new filter and pan gasket. Drop the pan clean it out, replace the filter put the pan back on and put the new fluid in. If it runs better you may have solved your problem. If not, the trans may beed rebuilding soon. Brown fluid is a tell tail sign usually. Just make sure the fluid is always bright red.
