Clunk And Jerk After Coast To Acceleration?

I am having a problem that only seems to occur from 45MPH or more and what is happening is if I get up to say 65 and then coast to any speed lower but more than 45 and then hit the gas the car will sometimes make a clunk noise and almost always jerks, also the harder I hit the gas the more pronounced the jerk is.... I'm not sure what it is but from what I can tell I don't think it would be U-Joints because I dont feel any vibrations when accelerating from stop whatsoever, I don't think its the rear diff because I cant seem to hear any humming or grinding or anything like that.... The transmission was rebuilt about a year and a half ago and probably has less than 12K miles on it so I hope its not something internal in the transmission... Does anyone have any suggestions of what it could be?

Check the trans and torque arm mounts.

Agree check the mounts. Transmission mount will rip and sometimes even looks visually good until you jack up the transmission some then they seperate.

Does it happen from when you're at a stop and accelerate hard? It would def. be the tranny mount, especially if you feel it hitting the floor.
