The one on the carb is used for the power brake booster.
The one on the intake manifold is used for the transmission vacuum modulator, and the HVAC reservoir (not sure if yours is the smaller one that goes under the cowl lip, or the larger ball that mounts in the fenderwell under the vapor canister.

But back to topic, what actually goes into the intake hole? A plug with two little vaccuum nipples on it? and what does one of these HVAC reservoirs look like? ill do some looking tonight and see what I can come up with.
And the carb fitting is strictly the brake booster?

The manifold fitting has male threads to screw into the manifold, and hose nipples coming out of it to hook up rubber (or plastic) vacuum lines. You can make one from hardware store brass fittings.
The carb port was only used for the vacuum booster on factory carb'd cars.
