Fan not working properly

Fan not working properly

How's it going, I look around on the forums but I couldn't find anything that was similar. Sorry if this problem has been solved before.

Anyways, when I idle for a little while my temp will go up to around 220 and than the fan will come on, usually. Sometimes, the fan will come on a couple of times when it reaches 220 but than the next time it won't, and sometimes it doesn't come on at all. I'll reach around 250 and I'll shut it off, turn the battery on and after a few minutes, than the fan will turn on.

What would be causing this?

a bad fan relay,bad fan switch,a bad connection...
very slight chance it's a bad fan motor(brushes worn
down)
