Installing a chip

When I brought my 91 Z28 it came with a custom chip but the guy did not tell me who it was from or what level it was. The chip was in a small brown box with the number 40054 on the side. The chip itself has a little red sticker on the chip with

I have done a search on the slp site and cannot find any reference.
Also what is the procedure for clearing the memory before installing the chip? Someone said turn the ignition on then disconnect the battery and this will clear the memory.
