Closed Loop After 4 seconds?

I have been using WIN ALDL to try to get some info on what is wrong with my car, it runs rough and won't idle when cold, the rough idle is pretty constant regardless of temp, but gets smoother somewhat after 5 minutes or so. I see from the logs I have that it is taking a little over 1 minute for the car to get up to temp, but the ECM is going into closed loop after only 4 seconds. Also odd to note is that "sensor BLM" which I'm not sure what it is starts climbing immediately after temp is reached, then periodically drops back to zero and starts again. Sensor BLM reads a constant 11 until temperature is reached at about 65 seconds after car is started and then climbs slowly to 250 about 3 and a half minutes later and drops to zero and starts climbing again. I did a fairly long log, over 40 minutes and looking back at it I see this happening over and over the entire time it is on, regardless of whether idling parked or driving at 60MPH. I see the Rich/Lean counter is mirroring the BLM sensor reading although I'm not sure what this is indicating. Any help would be appreciated, thanks.
