Not tight enough, apparently.
How did you tighten them down? Set the lash and then turn the set-screw?

just stating the obious here, but the rocker nuts backed off....thats what happened. as to why it happened, it could be a number of things
yes tightened lash then set the set screw new to poly locks

It's virtually impossible to get the set screw tight enough when you turn it alone. Turn the set screw at the same time as you put the last 1/8 or 1/4 turn or so on the nut when you set the lash.

Put a wrench over the rocker bolt then use a allen key, and you will hear to clicks. The poly locks are then locked. Dont be scared use some force. there a bitch to get to click.

sometimes fresh set-screws have a little machening flashing left on the end. use a file to get rid of the sharp edges. check your studs too.
yes checked clerances and coil bind and guide to retainer clearances
studs are not flat should i file them also rolled push rods on flat surface checked out ok got lucky on that except one intake pushrod onpass side it kindy broke replacements on way
