Brake Wear? Lots of noise...not sure...

Transmission: 700R4
Brake Wear? Lots of noise...not sure...

I'm guessing this is a brake problem, but just wanted some advice before i tackle the situation. Everytime i drive the car, at lower speeds, around 15-25mph, if i ride the brake halfway, with the pedal NOT all the way down, then my car makes a noise as if someone was holding a metal pipe to a circular object, and once every revolution that that object made, it would make a griding noise....i cant explain it any better than that...i am just wondering if anyone had this problem before or knows what it is. My guess its the brakes...its comming from the rear. BTW, it doesnt happen always....but MOST of the time...thanks for any help.
