Two hoses off of PCV valve? what is a control vac?

I have a hose splitter coming off my PCV valve... one hose goes to the intake manifold like it should, and the other goes to a small unmounted plastic canister that says Rochester Control Vac on the top. It has three hoses running off of it, two of which I have no idea where they go. I'm planning on replacing the PCV system, and can't find any information on this. Any ideas/help/links?

It's a charcoal canister purge control valve setup.

Here's a pic of the emissions routing sticker:

I'd like to find more detailed information about this system and the pieces. Part numbers, too, if possible. I need to replace the PCV valve, a number of these hoses, and a hose splitter due to oil leaks. Thanks!
