'92 Firebird dying at low RPM

My '92 Firebird is dying at low RPM's. It happens 99% of the time after I change gears. (it's automatic) So let's say I'm in park it will be fine, but if I put it in drive or reverse and go slow it will die. It feels weak and sorta rumbles out if you know what I mean. But if I gun it and give it some gas it won't die...I've even felt it very close to dying, but when I give it gas it umm, "recovers" It is very embarrassing and happens very often. I would really appreciate your guys help. Thank you.

It's called a lean stumble. Vacuum leak sounds reasonable.
