CO emissions are a by product of combustion process and result from incorrect air/fuel mixtures

CO emissions

It depends on where you are. Some counties in CO don't test at all. The major metro areas (DEN, COS) do. Denver has the highest restrictions, they dyno test anything newer than 1982. Colo Springs doesn't dyno, but they are looking at CO, NOX and that all your gear is there. (cat, air pump, cannister etc) See this link http://www.mv.state.co.us/emissions.html for all the details.

What he said. Anything newer than 1982 needs an e-test every 2 years, older EVERY year, in urban counties. Most counties outside metro denver use a simple idle and 2500 rpm test, at no load at idle. It's pretty easy to pass, assuming you car is operating right.

Visual test consists of checking for an air pump and cat in cars that are supposed to have one.

Also of note is that e-tests are done by private repair shops, NOT state run emissions centers. This means that a lot of shops get a little cute with getting cars to pass, and knowing the right people, it's pretty easy to get stuff taken care of. I'm not saying that you should just hack out all your stuff and find the guy who will smog it for a 50, because I think low emissions is a GOOD THING, but it is more relaxed than a lot of states in that regard.
