slave cylinder took a dump

OK so I got a T5 and for a few weeks sometimes when stopped it won't go into gear. Had to go to 2nd the 1st and it would go in. Yesterday it started doing it at every stop then finally I press the clutch pedal one last time and BAM!!! Puddle on ground and everything.

So for sure I have to get a new slave cylinder but do I need to get the whole setup cause of the hose? And what could have caused this. the clutch worked fine it's a Centerforce DF. and I checked the pivot it was nice'n tight. I don't want to put the new one in and BAM again. thanks
