clutch failure

long story short, my friend had his clutch replaced by dealer, it went out by slipping horribly in 2 months. we were going to replace it today and went to dig in and we found this inbetween the slave cylinder push rod and the clutch fork

It is part of the assembly, it belongs there, it did not cause the failure.
