Rebuilt tranny took another dump! Why?

Rebuilt tranny took another dump! Why?
My 700R4 was rebuilt 17,000 miles ago (@73,000 on the odo).
It was rebuilt by a guy in the area who has done excellent work for me in the past and has an untouchable reputation. When he did it, I had a Trans-go shift kit, Vette Servo and a tranny cooler installed, and the thing has crapped out on me twice.
Is there something else I should have him upgrade to keep this from happening again? I'm getting pissed having to rent a car every 5 months.

(I think I should have just bought a Pro-Built tranny the first time.)

When you switch trannys, you do flush the lines/radiator out where the tranny fluid goes, right? If not after a tranny blows, metal can get in there and if you dont flush it out it will get in the new one. metal chunks are a trannys worst enemy.
